Join the practice
To register with our practice, you’ll need to complete:
- Our New Patient Questionnaire
- Our New Patient Registration Form
These are available at reception, or you can complete them online using the links below. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct. You may wish to provide identification for registration with the practice but it is not required. We will not refuse to register someone because they cannot produce proof of identity, immigration status or proof of address. For more information on registration please see our registration form. If accepted you will be given an appointment with the Practice Nurse or Healthcare Assistant for a full health check. This is an opportunity to discuss any health needs or concerns you may have. Medical treatment is available from the date of registration. Please contact reception for further information.
Once your registration is accepted, your medical records will be transferred to us. Please note it can take several weeks for your records to arrive.
If you’re on regular medication
Book a 20-minute appointment with a GP soon after registering. This helps us ensure your ongoing care continues smoothly.

Ethnic Monitoring
Collecting information about patients’ ethnic backgrounds helps the NHS plan and provide the right services for our community. This is done through a simple tick-box on our forms.

Temporary Registrations
If you become unwell while away from home, or if you’re not registered with a GP but need to see one, you can get emergency treatment from a local GP practice for up to 14 days. After that, you’ll need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This means you’ll be added to the local practice’s list but will still remain registered with your usual GP. After three months, you’ll need to re-register as a temporary patient or join the local practice permanently.
To register temporarily, contact the GP practice you want to use. Practices don’t have to accept temporary patients, but they must offer emergency treatment. You can’t register as a temporary patient at a practice in the same area where you’re already registered.

These fact sheets explain how the UK’s National Health Service (NHS) works. They are designed for people who have recently moved to the country and/or who may be unfamiliar with how to access healthcare here.

They are written in clear, simple language and have been tested with user groups to make sure the information is easy to understand.

Access & Facilities
The main entrance has wheelchair access. We also have wheelchair accessible toilet facilities. There is a doorbell situated by the front door and if you need any help our team will be pleased to assist you.
There is limited car parking adjacent to the practice building. Please do not park in spaces reserved for our staff. We ask that you park sensibly and considerately as we are in a residential area. All cars are parked at the owners’ risk, so please take note of any on-street parking restrictions.
There is a bus stop outside the practice and is served by the number bus 60, 58 and 59 which stops on the Coventry Road. The nearest train station is Small Heath and is about a 10 minute walk.
