Some things that you may need to ask a doctor for are not included within NHS services, and are paid for as private work.  This includes:

• Private Sick Notes
• Insurance Reports
• Fitness To Travel Forms
• Holiday Cancellation Forms
• Private Medical Examinations
• Some travel vaccinations that are not covered by the NHS . 

If you require any of these services please ask our reception team. If you request one of these services you will be asked for advance payment for which you will be provided with a receipt.

An approximate waiting time for the completion of these types of forms will be given. 

These forms cannot be completed during a doctors’ consultation.
